You will receive an email confirmation when your form is received. If a refund is not requested by Friday, November 11, 2022 (4:00 PM) deadline you will receive credit on account for your next purchase at the State Theatre. Thank you for your patience.

If you have given or received the tickets as a gift, the original ticket purchaser must respond.
